Wellbeing Co-ordinator Service - Community Healthy Ageing Team

Age UK Cheshire has been awarded a contract worth £165,092.91 to provide a Wellbeing Co-ordinator service embedded within the Community Healthy Ageing Team at Countess of Chester Hospital NHS Foundation Trust. The service delivers non-clinical support to patients, primarily older adults, including emotional wellbeing support, advocacy, advice, and system navigation through face-to-face contact, telephone support, and home visits. This is a continuation of an existing service with a three-year term, aiming to improve patient outcomes, enhance independence, and reduce hospital admissions.
