Cheshire West and Chester Council is seeking a service provider for Community Home First, which supports timely hospital discharge of Pathway 0 and Pathway 1 patients back to their usual residence. The service will be delivered through Community Sector coordinators (Pathfinders) integrated within the hospital discharge team to triage and coordinate community and voluntary sector support, and will provide post-discharge community-based assistance for the first few weeks to support patient recovery at home.
