Housing, Health & Wellbeing Hospital Discharge Support Service

Durham County Council has awarded a contract worth £2,910,848 to WAYTHROUGH to deliver a Hospital Discharge Support Service aimed at preventing individuals aged 18 and over from being discharged into street homelessness. The service provides in-hospital assessment, advocacy, and discharge coordination alongside time-limited post-discharge support for individuals who are homeless or at risk of homelessness and have complex support needs related to drug and alcohol use or Adult Social Care involvement. The provider will work in partnership with health, housing, and social care services to ensure safe, timely, and sustainable discharges with appropriate accommodation and care pathways.
