Provision of Discharge to Assess Services (Night Services)

Somerset Council has awarded a contract worth £334,000 to C L Lifestyles LTD for the provision of Discharge to Assess (D2A) night services, a supported discharge pathway that enables people to return to their usual place of residence for assessment rather than remaining in acute hospital settings. The service provides personalised reablement focused on enabling individuals to regain independence, with assessment and care planning occurring in the person's home environment to facilitate faster discharge and prevent unnecessary long-term care placements.
