Healthy Lifestyle Case Management System

Portsmouth City Council has awarded a contract to Cyber Media Solutions Ltd for a Healthy Lifestyle Case Management System to support the Wellbeing Service in managing caseloads for residents addressing health risk factors including smoking, physical inactivity, obesity, hypertension, and alcohol consumption. The system will support screening and tracking of approximately 2,500 individuals annually referred from hospitals, GPs, and self-referrals, with outreach to high-prevalence groups including routine workers, deprived areas, and people with mental illness. The contract was awarded on 8 June 2026 and is now in delivery.
