ASC161 Sandwell Community Dementia Service

Sandwell Metropolitan Borough Council awarded a contract worth £1,314,428 to Communities in Sync Limited on 8 July 2026 to deliver a Community Dementia Support Service. The service, delivered through a provider collaborative model with sub-contractors, provides integrated, person-centred dementia support across community settings including care homes and hospitals, with a focus on proactive identification, care planning, and coordination with primary care networks, health teams, and social care. The service includes developing care plans, training for people living with dementia and their carers, professional development, and community awareness initiatives to support dementia-friendly practices across Sandwell.
