  The aim of this service is to facilitate recovery from illness and successful hospital discharge through the provision of high quality support in the home, so that people are confident and able to discharge from a hospital setting and live independently in the community. It is envisaged that this contract will be jointly funded with the Integrated Care Board (ICB) through the Section 75 budget.
