Auftragsgegenstand ist die an den aktuellen Leitlinien der Deutschen Gesellschaft für Kardiologie - Herz- und Kreislaufforschung e.V. (DGK) orientierte, die Regelversorgung und Disease Management-Programme ergänzende telemedizinische Betreuung von Versicherten der AOK Nordost mit Herzinsuffizienz im Rahmen eines spezialisierten und strukturierten Versorgungsprogrammes als Vertrag der besonderen Versorgung gemäß § 140a SGB V. Mittels telemetrischer Übertragung von Zeichen und Beschwerden des Erkrankungsbildes beim teilnehmenden Versicherten sowie der Auswertung und Beurteilung dieser Krankheitsdaten durch den Vertragspartner wird eine engmaschigere Betreuung und effektive medizinische Versorgung der teilnehmenden Versicherten gewährleistet.
