Overview
1. Przedmiotem zamówienia jest:
a. Dostawa zestawów do fotoferezy w ilości i wymaganiach określonych w Załączniku nr 3 do SWZ.
b. Dzierżawa Aparatów do fotoferezy pozaustrojowej w systemie zamkniętym - 2 szt. dla UCK, zgodnie z opisem zawartym w Załączniku nr 4 do SWZ.
2. Zamawiający dopuszcza możliwość składania ofert częściowych w ramach dwóch Pakietów. Pod pojęciem oferty częściowej rozumie się pojedyncze pakiety ustalone poniżej:
Pakiet 1 – Zestaw do fotoferezy pozaustrojowej do aparatu CellEx (Pakiet 1, Załącznik nr 3 do SWZ)
Pakiet 2 – Zestaw do fotoferezy pozaustrojowej (Pakiet 2, Załącznik nr 3 do SWZ) wraz
z dzierżawą 2 szt. Aparatów do fotoferezy pozaustrojowej w systemie zamkniętym (Załącznik nr 4 do SWZ);
Wykonawca może złożyć ofertę na jedną lub obie części.
