Overview
Versicherten mit schweren psychischen Erkrankungen soll ein bundesweites, wohnortunabhängiges digitales Nachsorgeangebot zur Verfügung gestellt werden. Rechtgrundlage für die vertragliche Vereinbarung mit einem zukünftigen Auftragnehmer (AN) bildet der Paragraf §140a SGB V. Das Angebot richtet sich an Erwachsene ab 18 Jahren, die erstmalig mit definierten psychischen Indikationen (Depressionen, Schizophrenie) aus dem Krankenhaus entlassen werden.
